Your child has received a blue folder with some important documents and forms for you to review, complete, sign and return to school with your child. The following forms were sent home:
Student and Family Handbook. Note: The last page of the handbook is the form.
Student Dismissal by Parent Pick Up Letter and Form.
Demographic Verification Form. Parents are to review carefully all the information. Make any corrections and include any other information such as phone numbers, email addresses, and emergency contacts. Please use a pen with blue or red ink.
Student Permission and Consent Form.
Student Emergency Card for the Health Office form.
Community Eligibility Provision (CEP) Household Income Eligibility Form 2024-2025.
Please complete the forms and return them to school with your child using the blue folder as soon as possible.
