Student Information Packet

If registering more than one child during your visit to the Registration Office, each child will need a completed packet.

Please print and return the completed Student Information Packet and Home Language Questionnaire to the Registration Office at the McKinley Administrative Bldg., 245 South Shore Blvd., Room 103.

**NOTE: The District is now requesting new documentation with regards to residency. Please review residency requirements on Page 2 of the information packet above.

A Student Information Packet is required for each Lackawanna City School District resident student attending a public, a non-public, or a charter school.

Property Search

Submit a completed and notarized student information packet for each child to the Registration Office. You will also need to provide additional documents as indicated in the packet.

Registration Office hours are Monday through Friday from 9 a.m. to 1 p.m. or call 716-821-5610 ext. 7703 to schedule an appointment.
