Non-Public or Charter Schools

Print and return the completed Student Information Packet, one for each child, to the Registration Office at the McKinley Administrative Bldg., 245 South Shore Blvd., Room 103. Office hours are 9 a.m. to 1 p.m. or call 716-821-5610 ext. 7703 to make an appointment.

**NOTE: The District is now requesting new documentation with regards to residency. Please review residency requirements on Page 2 of the information packet above or see below for a summary.




You will need to bring the following documents, which will be photocopied and returned to you:

  1. Original Birth Certificate of Registering Student 
    The Original Birth Certificate is required.
  2. Photo Identification of Parent/Guardian
    A valid driver's license with photo or non-driver's photo identification is required.
  3. Proof of Residency
    One Primary proof of residency and at least one Secondary proof of residency. See below for a list of acceptable Primary and Secondary proofs of residency.
  4. Legal Custody Papers
    Legal custody paperwork is required, if applicable.
  5. Legal Guardianship Papers
    Legal guardianship paperwork is required, if applicable.
  6. Immunization Record
    A Doctor's Certificate of Immunization and Physical Exam are required.
  7. Report Card
    Your child's most recent report card and transcript, if applicable.
  8. Individualized Education Program (IEP) or 504 Plan
    Your child's most recent IEP or 504 plan, if applicable.

All information packets should be completed prior to meeting with us. An appointment can be made by calling the Registrar, Tracy Welsh at 821-5610 ext. 7703.


List of Acceptable Primary and Secondary Proofs of Residency

It will be necessary for you to provide one Primary proof of residency and at least one Secondary proof of residency.

Acceptable Primary Proof of Residency:

  • Current year residential tax bill of an approved residential real property within the District in the name of the parent/legal guardian.

  • Residential mortgage statement instrument or legal statement showing ownership of an approved residential real property within the District in the name of the parent/legal guardian.

  • Lease or rental agreement of an approved residential real property within the District in the name of the parent/legal guardian. Must also include landlord’s name, address and telephone number for verification purposes. This document must have current dates, be signed and dated. *If there is no current lease or rental agreement, the Landlord/Property Owner Affidavit of Residency will need to be provided. This affidavit will need to be signed, dated, and notarized by the property owner*.

  • For instances where the child and the parent/legal guardian are living with another family member (owner of residential property), the family member will also need to provide 2 proofs of residency at the residential address within the District.

Acceptable Secondary Proof of Residency:

  • Utility bill (for service at a residential address within the District being billed in the name of the parent/legal guardian.

  • Utility Company letter to indicate service scheduled to begin within thirty (30) days at a residential address within the District being billed in the name of the parent/legal guardian.

  • Bank statement or pay stub in the name of a parent/legal guardian, addressed to a residential address within the District.

  • U.S. Postal Service verification of change of address to a residential address within the District, in the name of the parent/legal guardian.

  • Federal or NYS income tax documentation with preprinted name and address such as a W-2 Form, preprinted label from government or an income tax return with preprinted label. Documentation must be addressed in the name of the parent/legal guardian and addressed to a residential address within the District.

  • Insurance policy binder for homeowner or residential renter of any approved residential real property within the District in the name of the parent/legal guardian.

  • Certificate of Occupancy for residential real estate for real property within the District addressed and/or issued in the name of the parent/legal guardian.

  • State or other government-issued identification.

  • Voter registration document.

  • Documents issued by federal, state or local agencies (local social services agency, federal office or refugee resettlement).
  • Evidence of custody of the child, including but not limited to, judicial custody orders or guardianship papers.
