Lackawanna Public Schools

Lackawanna Public Schools Student Information Packet includes documents needed for registering.
 
If registering more than one child during your visit to the Registration Office, each child will need a complete packet.
 
Please print and return the completed Student Information Packet and Home Language Questionnaire to the Registration Office at McKinley Administrative Bldg, 245 South Shore Blvd, Room 103.
 

You will also need to bring the following documents, which will be photocopied and returned to you:

   a. Original Birth Certificate of Registering Student
The Original Birth Certificate is required.
   b. Photo Identification of Parent/Guardian
A valid driver’s license with photo or non-driver’s photo identification is required.
   c. Proof of Residency
One Primary proof of residency and at least one Secondary proof of residency. See List of Acceptable Primary and Secondary Proof of Residency. 
   d. Legal Custody Papers
Legal custody paperwork is required.
   e. Legal Guardianship Papers
Legal guardianship paperwork is required.
   f. Immunization Record
A Doctor’s Certificate of Immunization and Physical Exam are required.
   g. Report Card
Your child’s most recent report card and transcript, if applicable.
   h.
Individualized Education Program (IEP) or 504 Plan
Your child's most recent IEP or 504 plan, if applicable. 
 
All information packets should be completed prior to meeting with us. An appointment can be made by calling the Registrar, Dawn Polanski at 827-6703.
 
List of Acceptable Primary and Secondary Proof of Residency

It will be necessary for you to provide one Primary proof of residency and at least one Secondary proof of residency.

Acceptable Primary Proof of Residency:

A. Current year residential tax bill of an approved residential real property within the District in the name of the parent or Legal Guardian.
B. Lease or Rental Agreement of an approved residential real property within the District in the name of a parent or Legal Guardian. Must also include landlord’s name, address and telephone number for verification purposes.
C. Residential mortgage statement instrument or Legal statement showing “intent to purchase” an approved residential real property within the District in the name of a parent or Legal Guardian.

Acceptable Secondary Proof of Residency:

D. Utility bill (electricity, land line telephone, water/sewer or natural gas or propane) for service at a residential address within the District being billed in the name of a parent or Legal Guardian.
E. Utility company letter (electricity, land line telephone, water/sewer or natural gas or propane) to indicate service scheduled to begin within thirty (30) days at a residential address within the District being billed in the name of a parent or Legal Guardian.
F. Bank statement in the name of a parent or Legal Guardian, addressed to a residential address within the District.
G. U.S. Postal Service verification of change of address to a residential address within the district, in the name of a parent or Legal Guardian.
H. Federal or NYS income tax documentation with preprinted name and address such as a W-2 Form, preprinted label from government or an income tax return with preprinted label. Documentation must be addressed in the name of a parent or Legal Guardian and addressed to a residential address within the District.
I. Certificate of occupancy for residential real estate for real property within the District addressed and/or issued in the name of a parent or Legal Guardian.
J. Insurance policy binder for homeowner or residential renter of an approved residential real property within the District in the name of a parent or Legal Guardian.
